Summary

Approximately 26.7% of autistic children are non-verbal or minimally verbal. For these people, and for many others who find verbal communication effortful or unreliable, AI-enhanced communication tools represent one of the most genuinely promising applications of artificial intelligence to neurodivergent life. But the promise comes with conditions: who designs the tools, whose voice they amplify, and whether they help the person communicate or teach the person to perform neurotypical communication.

What the evidence shows

AI-enhanced AAC

Augmentative and alternative communication (AAC) — communication boards, speech-generating devices, symbol-based systems — has been available for decades. AI is now transforming these systems in several ways.

SpeakFaster (Google Research/Team Gleason, published in Nature Communications, November 2024) uses fine-tuned LLMs for eye-gaze text entry, achieving text-entry rates 29–60% above traditional baselines. The system saves 57% more motor actions than conventional predictive keyboards through context-aware abbreviation expansion — “gmhay” becomes “good morning, how are you?” It combines two LLMs: one for abbreviation expansion and one for contextual word prediction.

Traditional AAC prediction uses n-gram models that consider only the previous few words. LLM-based prediction leverages hundreds of preceding words and conversational history, producing dramatically better suggestions. This is not a marginal improvement — it changes what is communicatively possible within the time and effort constraints of motor-impaired communication.

Commercial AAC apps are integrating AI: Speech Assistant AAC now offers ElevenLabs voice cloning, Predictable uses personalised learning from user patterns, and AACessTalk uses LLM-based mediation to structure turn-taking and reduce cognitive strain.

LLMs as conversation partners

Research (2023–2025) documents autistic people using general-purpose LLMs (ChatGPT, Claude) for drafting emails, interpreting social situations, practising conversations, and navigating neurotypical social expectations. LLMs operate on text only, eliminating the need to process simultaneous non-verbal communication—a significant accessibility feature for those more comfortable with purely written exchange.

Therapists and experts note that LLM responses can be “overly wordy, vague, and potentially overwhelming,” so raw LLM output needs filtering or refinement. Concerns exist about LLMs amplifying social withdrawal or triggering rejection sensitivity without safeguards. A 2025 study surveying 200 autistic adults found the relationship is a “double-edged sword”: helpful for communication support but potentially isolating if it replaces human connection entirely.

Voice synthesis

Apple Personal Voice (2023) allows users to record 150 sentences; machine learning processes the voice overnight on-device to create a personalised synthetic voice for FaceTime, calls, and AAC apps. Google Project Relate (2021, ongoing) transcribes non-standard speech to text and allows re-statement using clear synthesised speech. Both are designed for people whose speech is difficult for others to understand, but neither has been specifically validated with autistic populations—most evidence comes from ALS and aphasia research.

The abandonment problem

30–50% of AAC users abandon their systems. Documented causes include high cost, cognitive overload in navigation, mismatch between system vocabulary and actual communication needs, and social stigma. AI may reduce abandonment through better prediction (less manual entry), contextual adaptation (vocabulary that matches the situation), and personalised learning (systems that adapt to the individual’s patterns over time). But the core barrier — systems designed without the people who use them — remains.

The “speaking for” problem

When AI predicts what someone wants to say, whose voice is it? This is the central ethical tension in AI-enhanced AAC. If systems are trained on neurotypical language patterns, they may predict what the system thinks an autistic person should say rather than what they want to say. Word prediction risks homogenising neurodivergent communication styles. The person must always be able to override, edit, or reject predictions, and the system must learn from those rejections.

Privacy is a parallel concern: AAC data sent to cloud-based LLM servers may be used to train commercial models without informed consent. For people who communicate through AAC, their communication data is their voice—and it deserves the same protection as any other personal data.

The intellectual disability dimension

AI-enhanced AAC for people with both autism and intellectual disability is almost unstudied. Symbol-based systems (graphic symbols, photographs) remain primary for this population. AI needs to support, not replace, these modalities through better symbol prediction, simplified interfaces, and communication partner guidance. AACessTalk’s approach (structuring turn-taking, offering tailored vocabulary suggestions) shows potential, but dedicated research with ID populations is essentially absent.

Open questions

Can LLM-based AAC be initialised for someone with no existing communication corpus (the “cold start” problem)? Current approaches use pre-trained models, but autism-specific vocabulary initialisation remains unstudied.

How do we ensure AI communication tools amplify the person’s voice rather than shaping it? This is a design question, an ethics question, and a power question with no settled answer.

What happens to AAC user data? Privacy frameworks for AI-assisted communication are weak, and the people most affected have the least power to advocate for their own data rights.

Implications for practice

AI-enhanced AAC represents genuine progress for people who use communication aids. If you support someone who uses AAC, explore whether AI-enhanced options might reduce their communication effort and increase their expressive range.

For any AI communication tool: check whether it was designed with its intended users. If not, treat its predictions and suggestions with appropriate scepticism — it may be predicting neurotypical communication, not the person’s actual intent.

Voice synthesis and voice banking should be considered proactively for anyone who may lose speech, and should be offered as a positive choice rather than a crisis response.
